According to the regional characteristic, states in which poverty rates increased the most included Florida, Illinois, Utah, South Carolina and others. It is also necessary to analyze the dynamics of poverty rates among different races. According to the survey, poverty rates increased greatly among all of the races present in the United States. At the same time, the income level of White, Asian and Black households remained unchanged while the income of Hispanic households experienced some decrease. Among these groups, Black households have the lowest level of income and thus have the highest poverty rates among the groups in the society. In 2003, the median income of Black families reached only $30,000 which was only 62% of the $48,000 income of White households (non-Hispanic origin). The group with the highest income level, and thus the smallest number of people below the poverty line was Asians. Their average income reached $55,000 in 2003. It is possible to conclude that among all of the groups, Blacks are experiencing the most problems with obtaining income. Asian households receive almost the double income of what Black households receive. Due to the fact that poverty rates are increasing with years, it means that Black citizens are going to become more and more vulnerable in the society. There are many reasons why the poverty rate among Blacks has changed the most during the recent years. The experience of New Orleans shows that despite the hopes of African Americans hat discrimination would be reduces with time, this process is very slow. Most of the people affected by hurricane in New Orleans lost their houses and jobs, and it will be very difficult for them to achieve their previous level of income in the following years. Unfortunately, the government did not take any measures to assist the people whose houses were devastated by the hurricane. Situation with New Orleans is only one of the examples of how discrimination occurs in the United States and Black population is unable to reach the same income level as other races. Another reason of high poverty rate among Blacks is their inability to enter prestigious establishments of higher learning. Due to the low income level of their families in comparison with other races, they are often unable to obtain good education which would lead them to high-paying jobs. At the same time, it is necessary to mark that due to the introduction of Civil Rights legislation, the poverty level of Blacks in the recent years is much lower than it was before the adoption of legislation. Even though the poverty rate has been increasing during the last years, the average rate is much lower than it was in the 50s. Despite the fact that Blacks are still experiencing some sort of discrimination, it is much less noticeable as it was before the adoption of Civil Rights legislation. Owing to the legislation, they finally got an opportunity to have the same rights as white people and thus the overall poverty level among Blacks declined dramatically in comparison with previous decades. The Water Clock in the Tower of the Winds Essay Noble and Prices in-depth article examining the specific technical attributes of the water clock in the Tower of Winds in the Roman Agora of Athens purports to be a virtual reconstruction of the Tower of Winds and, specifically, the water-clock and supporting water-tower within. The article is just that and little else. While steadfastly maintaining through out the article that the water-clock and the Tower of Winds have received too little literary and scientific attention over the centuries and lamenting that such a wonderfully curious structure (which has been maintained and continuously occupied over the many centuries since its construction), the authors do little to dramatize the Towers existence or bring the rich archaeological evidence and information made available by the structure into vivid, documentary realization. True enough, the Tower and the water-clock are generally passed over by scholars. An example is G. J. Whitrows mention of the Tower of Winds in his book Time in History: Views of Time from Prehistory to the Present Day which states simply: there is evidence of more elaborate instrumentation, such as the Tower of the Winds which can still be seen in Athens, north of the Acropolis. Designed and built by the astronomer Andronicus Kyrrhestes of Macedonia in the second quarter of the first century BC, with a wind vane and complicated sundials on each of its eight walls, its most interesting feature is a reservoir in a smaller building that stood next to its south side 1 with the implication that the rest of the structure was, in fact, of little interest. Ironically, the very concept of a water-clock provokes a sense of mystery and interest. The origin of water-clocks is presumed to be Egyptians who developed the water-clock as a method for keeping track of time at night when sun-dials were, obviously, incapable of functioning: To provide a means of measuring time at night the Egyptians also invented the water-clock, or clepsydra as the Greeks later called it Vitruvius, writing about 30 BC, described a number of types 2 so the history of the water-clock is deep and richly extensive. The Noble-Price article hints at this rich history for the technological and cultural significance of the water-clock, but evades any real crystallization of the possible incarnations of water-clocks which preceded the massive example in the Tower of Winds. Before examining the strengths and weaknesses of the articles largely technical explication, it may be useful to summarize in general what a water-clock is and what it is intended to do. Although the Noble-Price article certainly fulfills this need for preliminary information, the Columbia Encyclopedia does a far better job of stating, succinctly, the overall historical evolution of the water-clock: More elaborate clepsydras were later developed. Some were double vessels, the larger one below containing a float that rose with the water and marked the hours on a scale. A form more closely foreshadowing the clock had a cord fastened to the float so that it turned a wheel, whose movement indicated the time. A further step was the use of gear wheels and a turning pointer. 3 Another key point of the Noble-Price article relevant to the history of clepsydras is that which demonstrates the distinction between the two major types of water-clocks (or clepsydras) which were used extensively in ancient times: the outflow model and the in-flow model. The article mentions that The outflow clepsydra was known as early as the third millennium B. C. in Egypt, [ ] In this type water is allowed to escape from a vessel by dripping from an orifice near its base. Time is then calculated by measuring the fall of the water level, or the entire period taken for all the water to drain away with the subsequent negative outcome that the rate of flow through the orifice depends on the head of water above it; therefore as the water drains away the flow becomes slower. 4 By contrast, with the inflow device, the invention of which Vitruvius ascribes to Ktesibios, water was fed into a tank somehow equipped to provide a constant head of water. From a small orifice near the bottom of the tank, water dripped at a constant rate into a cylindrical container provided with a float; the float indicated the change in water level and therefore the time elapsed 5 and this solution si evident in the Tower of Winds where The cylindrical tower section of the Tower of the Winds is perfectly suited to house such an apparatus 6. Such technical distinctions are seemingly minor, but play a key role in the pursuit of the article under discussion. As Noble-Price indicate in the articles opening paragraph, the intention of the article is to fill this long-standing lacuna 7 where the captivating archaeological evidence of The Tower of Winds is at long last brought to the attention of serious observers by way of the authors practical restoration of the mechanism designed by Andronikos of Kyrrhos in Macedonia, probably near the beginning of the second half of the first century 8 and the authors certainly fulfill this promise. The shortcoming of the article is its dry, technical explication of the water-clock and Tower which does little to improve upon the authors own dry observation that At the outset it must be admitted that literary and historical allusions to the Tower of the Winds or its designer give almost no indication that the building was anything more than an elaborate wind-vane. 9 By contrast, Suzanne Youngs study, An Athenian Clepsydra, narrates an engaging technical explication along with a dramatic recreation of the function of the clepsydra in the practice of ancient law: Our earliest authority for the clepsydra is Aristophanes. One of his chorus of old Acharnians grumbles that it is hardly fitting that youngsters should shame an old mans grey hairs by dragging him into litigation to destroy him at the clepsydra. 10 Young breaks up her technical explication with historical anecdote and humor: In a slightly later play he teases a jury-court veteran (his chief Wasp ) who never sleeps a wink11 or if he doze off the least bit his mind goes fluttering in the night about the clepsydra. Such an approach finds the reader far more prepared to absorb the more demanding specifications of the technical and cultural attributes under discussion. Similarly, Henry Robinsons article The Tower of the Winds and the Roman Market-Place adopts a narrative-historical approach to the interpretation of the water-clock and Tower, concluding that The Tower of the Winds served as a public time-piece for the city of Athens. Its interior instruments, then, like those in the horologium of Scipio Xasica at Rome, should have been accessible to the populace of the city at all times, both night and day. The absence of one cutting on each threshold block and of one on the floor of the Tower indicates that this was the case 12 and leading the reader to understand the cultural significance of the architecture. Perhaps the Noble-Price article might have benefited from a bit of personal introspection or commentary from the authors outside of that which pertains to the dearth of active scholarship in relation to their chosen subject for the article. Unfortunately, the authors vividly miss any opportunity to include such material, or even a bit of comic relief as is evident in Carl W. Blegans article Prosymna: Remains of Post-Mycenaean Date which catelogs a tremendous amount of information on a dizzying array of artifacts and manages to compress this information into an entertaining and memorable package. Perhaps it is necessary for the human mind to punctuate its absorption of information with cutaway moments of emotional reflection, humor, and introspection. If so, the Noble-Price article suffers greatly from its lack of such punctuated emotion. The Blegen article, for example, finishes with a bit of humor and mystery, describing an ancient Egyptian artifact: The arcs and the lines are for the most part accurately and carefully drawn. The zodiacal names and the numerals of the hours are written in fairly well formed letters which seem to belong to the end of the second century B. c. , and we may conclude that the sphere was made about that time. The dedicatory inscription, on the other hand, with its crowded, badly shaped letters, regular use of t for Z, EI for 0, and for R,must be a much later addition, perhaps assignable to the second century A. D. whatever its earlier history, the sun-dial was presumably at that time dedicated to Hera and set up in the sanctuary; from which it must subsequently have rolled down the hill to the place where it was found. 13 In conclusion, while the Noble-Price article does an admirable job of presenting technical details, is deeply researched, and honestly intended, the article lacks any narrative or dramatic intensity which and will likely do little to exhume the Tower of the Winds from scholarly or popular obscurity. Strengths and Weaknesses of Incident Reporting in Nursing Problem based learning. Using the nursing process as a model formulate an action plan by critically analysing the strengths and weaknesses of incident reporting in relation to nursing. Identify reasons for a low level of incident reporting and offer a solution to the problem, eg. awareness of incident reporting procedures, accountability, consequences of not reporting incidents. Include professional, ethical and legal practice as it relates to the role of the nurse: continuing professional development following registration. Introduction Problem solving in nursing is often aligned to the ‘nursing process’, which is the APIE model – assessment, planning, implementation and evaluation. Using this structure in problem solving allows the nurse to assess the problem, by gathering information about it, plan how to address the problem, and also plan how to implement change and evaluate the process. It is important to use structured approaches to problem solving which include evaluation so that the problem can be reassessed and actions changed accordingly. In this essay, the problem of low levels of incident reporting in clinical practice will be explored, through a review of the literature, and an action plan will be developed which is in line with the professional, legal and ethical imperatives which underlie nursing practice. It is important to identify the problem carefully, understand the potential reasons for the problem, and then to use this information to plan changes in practice which will allow the problem to be resolved, partially or fully. Discussion Assessment Incident reporting serves a number of purposes within clinical practice. It is part of the risk management process, for example, and is focused on improving patient safety (Wagner et al, 2004). â€Å"Incident reporting captures more contextual information about incidents, and, when actively promoted within the clinical setting, it can detect more preventable adverse events than medical record review† (Evans et al, 2006 p 40). Taylor et al (2004) show that reporting allows for the understanding of and prevention of medical and healthcare errors, and provides data on the occurrence, frequency, types and results of such events. Incident reports however can also be used for â€Å"personnel credentialing and review (Taylor et al, 2004). According to Waring (2005) â€Å"the management of errors requires an acceptance of error with consideration given to the relationship between individual human behaviour and the factors that influence this behaviour† (p 1928). This means that the organisation must be responsive to incident reporting (Waring, 2005). However, incident reporting activities remain at low rates in many clinical areas, or do not report all of the incidents or the range of incidents that occur (Nuckols et al, 2007). There are limitations to what the incident reporting systems seem to be able to achieve in practice. â€Å"The subjective nature of reports, the lack of consistency and validation of incident data classification, and underreporting constrain incident reporting from being used as a reliable epidemiological tool to measure the frequency of events and whether interventions are effective in improving patient safety† (Evans et al, 2006 p 40). What this shows is that incident reporting is a very important aspect of the ongoing surveillance and improvement of healthcare practice. This underreporting may be due to a number of reasons, including a reluctance to admit mistakes or to be seen as being ‘to blame’, because of a sense of vulnerability on the part of nurses and other professionals (Bolsin et al, 2005). Johnstone and Kanitsaki (2006) state that as disciplinary and legal cases against nursing staff show, reporting nursing-related errors carries a risk for the nurses themselves, which is something which should be addressed in relation to promoting better incident reporting. It may also, in relation to medical professionals, be because a sense of medical hegemony which leaves doctors feeling invulnerable (Bolsin et al, 2005). However, the NMC (2008) requires nurses to be accountable for their own practice, to put patient needs and safety at the centre of all their actions, and to strive for the highest possible standards of care. Therefore, incident reporting must form a part of good nursing practice. Even the sense of vulnerability that can come from working in unsupportive clinical environments should not erase the legal obligation to report incidents which have adversely affected patients. Organisational culture may limit the incident reporting of the staff involved (Waring, 2005). Also, misconceptions of what constitutes an error may limit reporting activities (Throckmorton and Etchegaray, 2007). However the literature also suggests other reasons might prevent nurses from reporting adverse clinical incidents, despite the legal, professional and moral imperative to do so. One reason might be that the process of incident reporting is seen as too onerous. â€Å"Traditional narrative methods of documenting adverse incidents are time consuming and may not yield sufficient and accurate data.† (Wagner et al, 2004 p 835). Evans et al (2006) suggest that those incidents which occur most frequently, as well as incidents which are not easy to record using the typical recording systems seen in many clinical areas. Evans et al (2006) in their research found that barriers to incident reporting for doctors included a lack of feedback from reporting incidents previously, the form used taking far too long to complete, along with a belief that the incidents not reported were too trivial to be reported. Nurses, however, although they also found a lack of feedback prevented them reporting incidents, but unlike doctors, they believed that there was no point in reporting near misses, and they quite often forgot to make a report when the ward was too busy (Evans et al, 2006). Incident reporting can be related to specific clinical risks for target populations, groups or clinical areas. For example, incident reporting on falls in elderly care settings is a high priority (Wagner et al, 2004). It is also commonly a priority in relation to medication errors (Handler et al, 2007). The solutions to this problem are potentially diverse. The literature shows that for some clinical areas, for example, changing the documentation to suit the area and the kinds of incidents being reported might improve incident reporting rates and reduce incident rates (Wagner et al, 2004). Thus, providing streamlined, focused documentation might be a solution, but this would have to be quite specific, and some clinical areas might contain too many diverse risks for this. Vogus and Sutcliffe (2007) suggest a more managerial approach to improving incident reporting rates, such as bundling safety organizing with leadership (trust in manager) and design (use of care pathways) factors into nursing practice. Their research suggests that managerial approaches which affect the everyday behaviours of nurses can improve incident reporting (Vogus and Sutcliffe, 2007). Handler et al (2007) in their research showed that organizational-level interventions rather than individual-level interventions improved medication error reporting. Nakajima et al (2005) suggest that incident reporting should form part of patient safety programmes, and suggest the use of a web-based incident reporting system, along with identified staff responsible for incident reporting and support, staff education programmes, and integrated (and varied) feedback mechanisms. They found that this mulit-layered approach helped to improve and support a safety culture, improve multidisciplinary collaboration, and an overall systemic improvement, but that this required strong managerial leadership (Nakajima et al, 2005). Evans et al (2007) also used a similar approach, with identified people at all levels of staffing who supported the process, tailored incident reporting systems, and staff education. Kingston et al (2004) focus on the use of all kinds of supportive approaches and mechanisms to basically motivate staff to report errors. â€Å" Both medical and nursing participants made comments that a more effective and efficient incident reporting system without threat or blame, providing prompt, relevant feedback and driving improvements in health administration, would possibly motivate medical staff to report† (p 38). Pierson et al (2007) state that â€Å"web-based or electronic error reporting systems are particularly effective in increasing the quantity and quality fo reporting and yielding the type of information needed for improving care† (p 297). This would suggest that the use of some kind of electronic system would be an appropriate way of improving incident reporting, although this author believes this might depend on the skills and attitudes of staff concerned. Taylor et al (2004) cite similar findings from their research, suggesting that electronic reporting systems could make it easier and quicker, and state that â€Å"a substantial educational effort, aimed at nurses and, in particular, physicians, about which types of events should be reported and how to report errors is needed† (p 734). Dollarhide et al (2007) show the efficacy of a handheld-based electronic reporting tool for clinical incident reporting, showing that this made reporting much easier and more streamlined , but these would be too costly for the NHS setting. Planning The change that would be needed is summarised below. The change would be planned to incorporate the following elements: Identification of key personnel, including managerial level leaders, and staff across each grade and within each profession, to support the patient safety programme. Development of a mission statement encompassing patient safety protocols and incident reporting processes Development of incident reporting tools for specific incident reports, and an electronic/digital/web-based or email-available general reporting form. Development and implementation of a staff education programme on the new system, and incorporation of training and updating on staff mandatory study days (attended yearly). Development of a range of feedback mechanisms to staff are aware of what happens to the cases that are reported upon. Implementation All of the above activities should be finalised before the change is implemented. The key personnel will help to disseminate the information about the change, and then implement the change, supporting staff constantly as they adjust to the new procedures for reporting. Evaluation Evaluation of the change should be formalised at six monthly intervals for the first two years, but should also be built in to the feedback and reporting mechanisms so that staff themselves are fully involved in the evaluation and can feel that they ‘own’ this aspect of their work. Involving staff at all levels is likely to improve motivation (Evans et al, 2007). Focusing on practical solutions which are effective in this environment means that the evaluation should look at suitability, effectiveness and acceptability by the staff concerned. Conclusion It would appear from this brief review of the literature that in order to improve incident reporting, it is necessary to motivate staff to report. This can be achieved through education, through better incident reporting tools, which may ideally be electronic, and through managerial level leadership which changes organisational/workplace culture as well as providing better staff  involvement, better feedback on reporting, and less threat to staff.
